Assisted Living and Residential Care Facilities must operate and provide services in compliance with all applicable State and local laws, regulations and codes.  This report reflects the findings of the complaint investigation conducted 07/13/2022.  The facility was evaluated for compliance with Oregon Administrative Rule 411, Division 54 and if applicable, Oregon Administrative Rule 411, Division 57.  The following deficiencies were identified:

Based on interview, observation and record review, it was confirmed that the facility is not providing a safe and home-like environment. Findings include:

Compliance specialist (CS) did not observe salon door unlocked with chemicals visible.  

Interview with Staff #3 and #4 on 07/13/22, they stated the salon door is never locked.  

Plan of Corrections:

Facility will have door locked.

Based on interview, observation, and record review, it was confirmed that the facility does not have enough staff working to meet the needs of the residents. Findings include:

Review of staffing schedules for July and August showed the facility has staff schedule.  No posted staffing plan.

Interviews on 07/13/2022, Staff #3-5 stated that there is not enough staff to meet the needs of the residents.  Currently there are 5 2-person transfers.  Residents are often left in the common areas with minimal supervision.  Staff can not get to daily activities with residents.

Plan of Correction: Facility will get a posted staffing plan and staff to the needs of the residents.
