It was determined the facility was in substantial compliance with the OARs 411-054-0030 for Residential Care and Assisted Living Facilities for Resident Services- Meals, and Oregon Health Service Food Sanitation Rules OARs 333-150-0000.

Details

Based on observation and interview, it was determined the facility failed to ensure food was prepared, and the kitchen was maintained, in accordance with the Food Sanitation Rules OAR 333-150-000. Findings include, but are not limited to:


Observations of the facility main kitchen, service kitchen, food storage areas, food preparation, and food service on 01/30/23 revealed splatters, spills, drips, and debris noted on:

 

- Interior of the ovens;

- Food containers in the dry storage area;

- Flooring throughout the kitchen;

- Open shelving and metal rack shelving;

- Legs of shelving throughout the kitchen;

- Underneath shelving and equipment throughout kitchen;

- Triple pot sink area;

- Dishwashing area including flooring, walls, and equipment;

- The hot cart used for food delivery;

- Interiors of drawers and cupboards in the service kitchen;

- Interior of the microwave in the service kitchen; and

- Exterior of cupboards in the service kitchen.


* There was no soap available at the hand washing sink.


* The cutting boards were deeply scored and stained.


* The metal prep table and shelving had peeling damaged shelf paper.


* Multiple bins had scoops with handles in the food.


* The chemical sanitizer for the low temperature dish machine was not monitored to ensure it was reaching the required level.


* Cheese was stored uncovered in the service kitchen refrigerator.


* Multiple cupboard shelves and the laminate counter in service kitchen were damaged creating uncleanable surfaces.


* A cupboard hinge was damaged and the door was falling off in the service kitchen.


* Caregiving staff, who assisted residents with toileting and incontinent care, were not using aprons while serving meals.


* Staff serving meals did not change gloves or wash hands between tasks.


The main kitchen was toured with Staff 2 (Kitchen Manager). He acknowledged the areas needing cleaning, repair and improper infection control practices.


The areas in need of cleaning and repair were reviewed with Staff 1 (Administrator). She acknowledged the above findings.

oOvens will be serviced and have a detailed clean. Completed on 2/10

oFlooring and metal racks: cleaning crew will do a deep clean, items will be added to cleaning schedule. Scheduled

oShelving mats that can be removed and washed have been ordered.

oDish area: cleaning crew will do a deep clean. Scheduled

oHot cart: cleaning crew will do a deep clean. Hot cart is also added to the cleaning schedule. Completed on 2/10

oService kitchen microwave is added to the cleaning schedule. Purchased new one on 2/16

oService kitchen will be professionally cleaned, including the drawers and cupboards.

oService kitchen will be professionally cleaned, including the exterior of the cupboards.

oSoap dispenser is being added to the hand washing sink area.

oNew cutting boards have been purchased. 2/13

oShelving mats that can be removed and washed have been ordered for all prep tables. Have been ordered.

oScoops have been removed from storage containers and training has been implemented. 1/31

oChemical sanitizer: test strips have been ordered. Delivered 2/13

oService kitchen will receive a deep clean, service staff is receiving training and taking food handler classes
