The findings of the re-visit to the kitchen inspection of 01/03/24, conducted 06/24/24, are documented in this report. It was determined the facility was in substantial compliance with the OARs 411-054-0030 for Residential Care and Assisted Living Facilities for Resident Services- Meals, and Oregon Health Service Food Sanitation Rules OARs 333-150-0000.

Details

Based on observation and interview, it was determined the facility failed to ensure kitchen practices and protocols were in accordance with the Food Sanitation Rules OARs 333-150-0000.


Findings include, but are not limited to:


On 01/03/24 at 11:00 am, the facility kitchen was observed to have the following concerns:


* The walk in refrigerator: three tier cart had uncovered dessert items on two of the three shelves and two sheet pans of uncovered jello and bean salad;


* The rolling cart next to convection oven:  three sheet pans with uncovered cupcakes;


* Garbage cans were uncovered when not in use;


* Food bins containing oatmeal, flour and brown sugar had lids with food debris on top;


* The reach in refrigerator and freezer bottom shelves had food debris and the exterior doors had drips/splatters and vents below the doors on the exteriors had dust/splatters;


* The window sill above floor mixer, ice machine and prep table had dust accumulation;


* The floor in dry storage had debris and the corner near the floor above cove base had black matter;


* The wall behind the stove and end of stove had grease drips/splatters; and


* The lower shelves of prep tables had drips/splatters/debris.  


The areas of concern were observed and discussed with Staff 1 (Dining Services Director) and discussed with Staff 2 (Executive Director) on 01/03/24. The findings were acknowledged.

Plan of Correction

1.  All food has been covered.  A new garage can with the

correct lid was ordered on 1/4/24.  All debris, drips, splatters, matter and dust have been cleaned/removed.


2.  The closing cook and dietary aide will ensure all food is covered and all debris, drips, splatters and matter is cleaned/removed.  The DSD will check for dust in the kitchen including the windowsill and the vents on the reach

in refrigerator and freezer.


3.  The opening cook and dietary aide with check daily that all food is covered and all debris, drips, splatters and matter have  been cleaned/removed.

The DSD will do weekly dust inspections.


4.  The DSD and ED will be responisble for seeing that the corrections are completed and monitored.
