OAR 411-054-0030 (1)(a) Resident Services Meals, Food Sanitation Rule (1) The residential care or assisted living facility must provide a minimum scope of services as follows: (a) Three daily nutritious, palatable meals with snacks available seven days a week, in accordance with the recommended dietary allowances found in the United States Department of Agriculture (USDA) guidelines, including seasonal fresh fruit and fresh vegetables; (A) Modified special diets that are appropriate to residents' needs and choices. The facility must encourage residents' involvement in developing menus. (B) Menus must be prepared at least one week in advance, and must be made available to all residents. Meal substitutions must be of similar nutritional value if a resident refuses a food that is served. Residents must be informed in advance of menu changes. (C) Food must be prepared and served in accordance with OAR 333-150-0000 (Food Sanitation Rules). This Rule is not met as evidenced by: Based on observation and interview, it was determined the facility failed to ensure the kitchen was maintained in accordance with the Food Sanitation Rules OAR 333-150-000. Findings include, but are not limited to: Observations of the facility kitchen, food storage, and food preparation areas on 12/03/24 noted the following in need of cleaning or repair: a. The following areas/items were found needing repair: * Steam table cupboards and shelves were chipped and had exposed wood; * Industrial mixer had chipped paint; * Multiple walls had holes, and a corner wall near the rear prep table was damaged; and * Multiple doors and/or door frames had chipped paint. b. The following kitchen areas/items were observed with spills, splatter, debris, black matter, and/or dust accumulation: * Multiple walls and floors throughout the kitchen; * Multiple stainless tables and shelves; * Steam table cupboards and shelves; * Hood vents above the stove and flat top; * Ceiling lights; * Industrial mixer; and * Blender. On 12/03/24, Staff 1 (Executive Director), Staff 2 (Dining Services Director), and the surveyor toured the kitchen and reviewed the areas in need of cleaning and repair. They acknowledged the findings.

Plan of Correction

Dietary Aides and Cooks inserviced on 12/10/2024 regarding food sanitation practices within the kitchen and dining areas. Cleaning schedules were reviewed, revised and again implemented. Daily, weekly, monthly, and quarterly cleaning tasks assigned according to schedule. Dining Services Director will review cleaning task sheets on each shift worked daily for 90 days then as needed ongoing. Maintenance Director has repaired all walls and shelves that had surfaces that were not considered to be "cleanable." Dining Services Director will report any surfaces to Maintenance at the time that they are considered to be not cleanable in order to maintain kitchen surfaces. Dining Services Director will inform Maintenance Director by entering it in the Maintenance Log in order to sign off when completed. This will occur as needed and ongoing for kitchen maintenance.
