The following was identified: a. The flooring in the kitchen was noted in need of repair and had cracks and breaks and large areas where the transition seal, between flooring material, was missing, worn, or peeling. At 12:08 pm, Staff 3 (Maintenance Director) reported the flooring had been worked on and resealed in the past and acknowledged the areas reviewed needed repair. b. The facility lacked the proper testing strips required to ensure proper sanitization was completed throughout the kitchen, including food contact and non-food contact surfaces and staff were unaware how to use test strips to check sanitizers. The need to ensure the kitchen was maintained in a sanitary manner and food was prepared and served in accordance with Food Sanitation Rules was reviewed with Staff 1 (Administrator) and Staff 2 on 08/19/25 at 12:51 pm. They acknowledged the findings.
