The findings of the first revisit to the kitchen inspection 10/02/23, conducted 12/07/23, are documented in this report. It was determined the facility was in substantial compliance with the OARs 411-054-0030 for Residential Care and Assisted Living Facilities for Resident Services- Meals, and Oregon Health Service Food Sanitation Rules OARs 333-150-0000.

Details

Based on observation and interview, it was determined the facility failed to ensure the kitchen was clean and in good repair in accordance with the Food Sanitation Rules OAR 333-150-000. Findings include, but are not limited to:


a. Observation of the kitchenette on 10/02/23 at 10:41 am revealed an accumulation of food spills, splatters, loose food debris, garbage, dirt and/or dust on or underneath the following:


* Flooring throughout the kitchenette;

* Walls throughout the kitchenette;

* Counters throughout the kitchenette;

* Shelving throughout the kitchenette;

* Juice machine;

* Steam table;

* Electric griddle;

* Toaster; and

* Ceiling ventilation units.


b. The following kitchen items needed repair:


* Trim of both doors had chipped paint;

* Ceiling vent above the steam table was not attached to the ceiling; and

* Juice machine had a leak which resulted in pooled liquid.


The areas in need of cleaning and repair were reviewed with Staff 1 (Assisted Living Manager) and Staff 3 (Nutritional Services Manager) on 10/02/23. They acknowledged the findings.

Plan of Correction

A cleaning checklist will be created and kept in the kitchen in the Assisted Living that will be checked off daily by dining staff or facility services staff once the following items have been cleaned: Flooring, walls, counters, shelving, steam table, toaster and ceiling ventilation units throughout the kitchenette. The electric griddle is no longer being used and has been removed from the kitchenette. The juice machine has been repaired and is no longer leaking. Cleaning of the juice machine will also be added to the checklist for dining to complete daily. Work orders have been entered for maintenance to repair the trim of both doors and to fix the celing vent above the steam table to ensure it is attached to the ceiling. Monthly sanitation walkthroughs will be completed by the Registered Dietitian and sent to the ALF Administrator for review and any necessary follow up.
