The findings of the first re-visit to the kitchen inspection of 05/16/23, conducted 08/23/23, are documented in this report. It was determined the facility was in substantial compliance with the OARs 411-054-0030 for Residential Care and Assisted Living Facilities for Resident Services- Meals, and Oregon Health Service Food Sanitation Rules OARs 333-150-0000.

Details

Based on observation and interview, it was determined the facility failed to ensure the kitchen was clean and in good repair in accordance with the Food Sanitation Rules OAR 333-150-000. Findings include, but are not limited to:


On 05/16/23 at 1:20 pm, the facility kitchen was observed to need cleaning and repair in the following areas:


a. Food spills, splatters, debris, dirt, and black matter was observed on or underneath the following:


* Beverage station in the scullery;

* Frames and blades of portable fans;

* Wall surrounding vents near the ceiling;

* Wall behind the ice machine;

* Interior of several cabinets;

* Metal hood over dish machine;

* Wall behind dish machine;

* Shelving underneath dish machine;

* Caulking in dish machine area;

* Floor perimeter, top edge of cove base, underneath appliances, and in grout lines between tiles;

* Ceiling pipes; and

* Interior of microwave.


b. The following areas needed repair:


* Scraped, gouged peeling paint on white shelves in the scullery;

* Scraped, gouged door and frame leading into the kitchen from the scullery;

* Several lower cabinets were missing doors and peeling Formica along the edges;

* A black strip of plastic in the dish machine area was peeling away from the wall;

* White strips of wall plastic was cracked or broken in several areas;

* The wall underneath the dish machine had cracked, peeling drywall and paint;

* The wall underneath the hand wash sink in the dish area had gouges, peeling paint and a hole in the drywall near the sink pipes;

* The rear green exit door had scraped paint on the frame and door, and a section of tile cove base next to the door was missing; and

* White, painted cabinets in the dry storage area had several scraped areas.


The areas that required cleaning and repair were observed and discussed with Staff 1 (Administrator), Staff 2 (Dietary Manager) and Staff 3 (Maintenance) on 05/16/23 at 11:30 am. They acknowledged the areas observed needed to be cleaned and repaired.

Plan of Correction

The POC for the cleaning of the kitchen: kitchen is staffing an extra employee for 16 hours a week to get caught up on the deep cleaning. A daily, weekly, and monthly cleaning schedule has been created. The cleaning schedule will be initialed by the staff when the task is completed. The dietary manager will ensure that is being completed.                     The POC for the repairs in the kitchen:  The shelving in the scullery is being replaced with metal racks. The damaged door frame and door leading into the kitchen will be repaired with new trim and repairing the damaged door, if unable to repair the door it will be replaced. The missing cabinet drawers and formica are being repaired and rehung, if unable to repair they will be replaced. The black strip of wall plastic will be replaced. The white strips of wall plastic will be replaced. The wall underneath the dish machine will be repaired. The wall underneath the hand washing sink will be repaired. The rear green exit door will be repaired if unable to repair it will be replaced. The tile cove will be replaced. The cabinets in the dry storage will be sanded down and repainted. If repairs are unable to be done in house we will be getting bids for the repairs to be done by a contractor. The kitchen manager and maintance manager will ensure that the repairs/replacements are completed and kept in good repair.
