Description: The facility failed to provide adequate care staff and effective administrative oversight regarding residents quality of care and services as evidenced by preliminary information from Community Based Care Licensing Complaint Unit. Also preliminary information from Local Office APS investigations indicate instances the facility failed report suspected abuse and neglect. A condition was placed on the facility's license requiring a restriction of admissions, staffing requirments, and traininig requirements. A new management company took over and the condition was amended on 01/18/18 allowing limited admissions of 1 every 7 days. On 10/1/18, a survey relicensure visit (#7XSC12) was completed and found facility to be in substantial compliance. Based on survey and recommendations from our community partners, the Department has determined that a license Condition is not warranted.
