Description: In August 2017 it was determined that the facility was not in substantial compliance with the Oregon Administrative Rules governing assisted living facilities and that the facility failed to enhance the dignity, independence and individuality of the residents. Due to the facility's noncompliance the Department imposed conditions on the facility's license that included repairing the exterior double entry door to the facility, contacting case managers as approopriate for assistance with behaviors and a registered dietician consultant.
