Description: Based on several recent substantiated investigations, as well as numerous pending Adult Protective Service investigations and information received by the Department, it has been determined that the Facility is not in substantial compliance with the Oregon Administrative Rules for Residential Care Facilities. Due to the facility's substantial noncompliance the Department placed the following conditions against the facility's license; restriction of admissions, RN Consultant, staffing requirements, pharmacy audit, RN and reporting requirements. On 12/18/17 the Department issued an amended license condition with the following terms; 1 new admit per 7/days with nurse consultant approval, RN Consultant, staffing requirements and reporting requirements.
