Description: The facility's failure to appropriately follow the Oregon Administrative Rules governing Residential Care Facilities placed residents at serious harm or risk of serious harm. Due to the facility's substantial noncompliance the Department placed the following conditions against the facility's license; Restriction of Admissions, RN Consultant, mandatory RN hours 40 p/week and reporting requirements. On May 30th, 2018, the Departments survey team completed a followup visit (5I3U14) at the facility, and found the facility to be back in substantial compliance. The condition was removed on 6/7/18.
