The findings demonstrate that the Facility is not in substantial compliance with the Oregon Administrative Rules for Residential Care Facilities and that the Facilitys noncompliance placed residents at harm and risk for harm. An immediate condition was imposed effective 3:30pm, June 19, 2018 with the following terms: Restriction of Admissions (ROA); Roundtheclock checks of outdoor courtyard; and Abuse Reporting Training.
