- A Residential Care Facility relicensure survey (#HL7J11) completed on January 16, 2019, at Brookdale Bend determined the facility was not in substantial compliance with Oregon Administrative Rules for Residential Care Facilities and that the facility's noncompliance placed residents at harm or risk of harm. The facility received twenty citations, two of which were at harm or potential for serious harm. Preliminary APS investigation report #00020254, with an incident date of February 17, 2019, indicates the following: The facility failed to provide a safe environment and to ensure resident safety. A facility staff member contacted local law enforcement regarding finding a bed bound and nonverbal resident covered in ants on his/her body and in his/her bedding. The facility has had an ongoing issue with ants in the facility and has had a monthly service contract with Terminex. Terminix has submitted a report to the facility that in order to resolve the ant issue, they would need to dig up flooring in order to more aggressively combat the ants. The facility has failed to follow this advice and did not contact Terminex after this resident was found covered in ants. The facility also failed to report this incident to Adult Protective Services. Terms include: 1) ROA; 2) Maintain RN/Admin consultant; 3) Comprehensive plan by professional pest control; 4) Comprehensive plan regarding resident safety monitoring; 5) RN training documentation; and 6) Reporting requirements.
