Description: The facility's failure to provide a safe environment and ensure caregivers were adequately trained, awake and available to meet the scheduled and unscheduled needs of facility residents placed residents at serious risk of harm and creates a situation that necessitates DHS to issue a license condition. In addition, DHS finds that the residents of the facility are at risk of immediate jeopardy. On May 15, 2019, the Department reviewed the information and representations submitted by the facility, the RN Consultant, agency partners and all facts related to the case and has determined, based on those representations, that the continued imposition of a license condition in this matter is not warranted and the condition was withdrawn.
