Description: Preliminary information provided to the Department by Agency Partners between June 4, 2019 and June 20, 2019, determined the facility was not in substantial compliance with Oregon Administrative Rule (OAR) 4110540025(1)(a) and (b); OAR 4110540055(1) and (2), and that the facility's noncompliance places residents at harm or risk of harm. Action items: Registered Nurse (RN) Consultant; Abuse reporting training and reporting requirement.On September 10, 2019, relicensure survey revisit #2 (M9U113) was completed and determined the facility to be in substantial compliance.On September 16, 2019, the Department contacted Agency Partners and it was determined there were no concerns with removing the Condition. Condition removed 09/19/2019.
