Findings: Witness #1 was care planned with aggressive behaviors and frequent agitation, and was known to yell at others that lead to altercations. On or about March 3, 2019, Witness #1 and the Alleged Victim had a non-injury altercation. The facility failed to implement interventions and re-direct residents away from Witness #1 which resulted in the altercation. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
