Findings: The facility failed to provide a safe medication system to ensure physician orders were followed for the Alleged Victim's medication. AV was to get a smaller dosage of the medication during the day and a larger dosage in the evening, however, these two dosages were switched in the AV's medication slot, which is maintained by facility staff. AV received incorrect dosages of the medication for approximately 18 days, which caused increased dizziness, placing AV at a risk for harm. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
