- Findings
- Alleged Victim (AV) had a known history of refusing staff assistance with hygiene, incontinence care and housekeeping. On or about March 21, 2019, AV was observed to have greasy hair that appeared wet, his/her clothes were soiled, AV had strong body odor and AV's nails were long with feces under them. AV's room was observed to have feces and urine on his/her bed, recliner, sofa, floor, shower chair, carpet, and a strong odor of urine and feces. AV developed redness to his/her buttocks and dry flaky skin as a result of not bathing properly. The facility failed to care plan for AV's known history of refusing staff assistance, which is a violation of resident rights, is neglect of care and constitutes abuse. **Change of Owner effective June 4, 2020: As the new owner of this facility, you are responsible for correcting any deficiencies which pre-date your ownership. You must correct violations which occurred under previous ownership, as directed by the Department. You will not be responsible for paying civil penalties incurred by previous owner(s). However, if you fail to correct identified deficiencies within the specified time you may be subject to aggravated civil penalties.
