Findings: On or about April 24, 2019, Witness #1 (W1) was found dragging the Alleged Victim (AV) by his/her hair in the hall, causing AV to cry out. W1 was known to have altercations and aggressive behaviors towards others and was to be “eyes on” by staff to prevent interactions. Staff did not follow the care plan, as there were not enough staff at the time to meet the scheduled and unscheduled needs of residents. The facility’s failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP20-00233 $188.00 fine assessed
