Findings: On or about April 29, 2019, Alleged Victim (AV) eloped from the facility, was found by the police, and brought back to the facility by the police. AV has a history of exit seeking and past elopements. The facility failed to appropriately care plan and failed to have appropriate interventions in place to prevent future elopements. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
