Findings: The facility failed to assess and intervene when the Alleged Victim (AV) experienced a change of condition. The AV had an event where he/she stopped breathing and was turning blue, however medical attention was not sought until days later, placing AV at a risk for harm. The facility's failures are a violation of resident rights, are considered neglect of care and constitute abuse.
