Findings: On or about June 5, 2019, Alleged Perpetrator 2 (AP2) attempted to transfer Alleged Victim (AV) by him/herself. AV fell through the opening of the shower chair, got stuck and ended up with multiple bruises to AV's breast, right abdomen, right buttocks, right thigh and upper forearms. AV had a similar incident on or about April 22, 2019, and an ISP was put into place requiring two staff persons to transfer AV. AP2 was aware of the two-person requirement as evident by AP2's signature of AV's ISP. AP2 failed to follow AV's care plan which is considered neglect of care and constitutes abuse. The facility failed to protect AV from neglect of care which is a violation of Oregon Administrative Rules.
