Findings: Alleged Victim (AV) had a known history of falls and was care planned for a tab alarm to be worn while in his/her wheel chair. On or about June 12, 2019, AV was found outside in the court yard and had fallen out of his/her wheelchair. The tab alarm was attached to his/her wheelchair inside the building and did not go off to alert staff AV was attempting to ambulate without assistance. AV sustained skin tears and re-opened skin tears from a fall the day prior. The facility failed to follow AV's care plan, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
