Findings: Alleged Victim (AV) was a known fall risk and experienced falls on or about the following days April 22, 2019, May 1, 2019, May 21, 2019, May 23, 2019, June 7, 2019, and June 12, 2019. On or about June 21, 2019, AV suffered a fall with head injury. It was discovered a small piece of linoleum was bubbled up and could be the cause of AV's numerous falls. The facility failed to care plan appropriately of AV's known history of falls which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: RCFCP20-00286 $1125.00 fine assessed
