- On or about May 31, 2019, Alleged Victim (AV) had money go missing from his/her room. The money was taken by an unknown individual. The unknown individual's actions are considered financial exploitation and constitutes abuse. The facility failed to provide a safe environment which is a violation of Oregon Administrative Rules.
