Findings: On or about June 30, 2019, Alleged Perpetrator 2 (AP2) administered the wrong medication to Alleged Victim (AV). The facility reported AP2 had a history of making medication errors. AP2 and the facility put AV at risk for serious harm which is a violation of resident rights, is neglect of care and constitutes abuse.
