Findings: On or about June 25, 2019, Alleged Victim (AV) moved into the facility and had a known history of falls. Between June 25, 2019 and June 28, 2019, AV suffered from four falls, once of which she/he fractured a bone. The facility failed to implement appropriate interventions according to AV's known history which is a violation of resident rights, is neglect of care and constitutes abuse. **Due to facility closure the Department will not be issuing a monetary penalty.**
