Findings: Alleged Victim (AV) is a fall risk and care planned to transferred into either the sofa or chair while watching TV in the living room. On or about July 8, 2019, AV was in the living room watching TV and slid out of his/her wheelchair. The facility failed to follow AV's care plan putting AV at risk for serious harm, which is a violation of resident rights, is neglect of care and constitutes abuse.
