Findings: On July 14, 2019, Alleged Victim (AV) was found to have passed but exact time of passing is unclear. One facility document indicated that a scheduled medication for AV was not given at 1600 due to AV’s passing. Another document indicated that Alleged Perpetrator 2 (AP2) administered an as-needed medication (0.25ML) to AV between 1615 and 1629 but was later reported that this was in error. The facility and AP2 were unable to account for the 0.25ML missing as-needed medication dosage. The allegation that AP2 failed to protect AV from financial exploitation was investigated and wrongdoing was unable to be determined. The facility failed to provide a safe medication administration system which is a violation of resident rights, is considered neglect of care and constitutes abuse.
