Findings: The facility and Alleged Perpetrator 2 (AP2) failed to provide appropriate services and follow the Alleged Victim's (AV) care plan when assisted him/her with transfers. The failure resulted in AV experiencing a fall while being transferred and was transported to the hospital with injury, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
