- Findings
- Alleged Perpetrator 2 (AP2) admitted that he/she diluted Alleged Victim's (AV) liquid pain medication with water which placed AV at risk of potential harm. AP2's actions is considered neglect which constitutes abuse. The allegation that AP2 financially exploited AV was investigated and findings were inconclusive. The facility failed to provide a safe environment for AV which is a violation of Oregon Administrative Rules.
