Findings: Alleged Victim (AV) needs staff assistance with eating. On or about June 28, 2020, AV was served a hotdog in a bun that was not prepared to AV's diet orders. AV choked on the hotdog putting AV at risk for significant harm. The facility failed to provide food prepared according to AV's diet orders which is a violation of resident rights, is neglect of care and constitutes abuse.
