Findings: Witness 1 (W1) was care planned with aggressive behaviors and had a known history of striking staff and other residents. On or about July 26, 2019, W1 was exhibiting behaviors when Alleged Victim (AV) and W1 were involved in an altercation resulting in W1 slapping AV in the face. The facility failed to provide a safe environment for AV which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP20-00312 $188.00 fine assessed
