Findings: The facility failed to provide a safe medication administration system and protect the Alleged Victim (AV) financial exploitation. On or about August 3, 2019, it was discovered by staff that the Alleged Victim's (AV) pain control medication had been swapped with another medication that AV does not take. The last person to sign out medications on the MAR was Alleged Perpetrator #2 (AP2). AP2 refused to take a drug test and left the facility when asked. The facility was aware that AP2 had a history of substance abuse, however, AP2 was rehired despite the facility's knowledge. AP2's failure is a violation of resident rights, is considered financial exploitation and constitutes abuse.
