Findings: Alleged Victim (AV) had a known history of falls and was care planned for a wedge cushion and a non-skid pad. Documentation indicates the wedge cushion was slipping off of AV's chair frequently. On or about August 5, 2019, AV fell out of his/her wheelchair due to the wedge cushion slipping and sustained injury. The non-skid pad was not in place at the time of the fall. The facility failed to provide a safe environment and failed to follow the care plan which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP20-00001 $375.00 fine assessed
