- On or about August 14, 2019, Alleged Victim (AV) had money go missing from his/her room. The money was taken by an unknown perpetrator which is considered financial exploitation and constitutes abuse. The facility failed to provide a safe environment which is a violation of Oregon Administrative Rules.
