Findings: On or about August 18, 2019, Alleged Victim (AV) was found on the floor with no injuries on day shift. AV was assisted back to bed. The facility did not complete any investigation around the fall and/or implement any interventions. On August 18, 2020, at approxemetly 10:10pm, AV was found on the floor and assisted up and back to bed with a gait-belt. Staff report there was injuries however they were not reported and they were not documented. The facility failed to access and intervene regarding AV's falls, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: RCFCP20-01031 $375.00 fine assessed
