Findings: The facility failed to provide a safe environment for the Alleged Victim during his/her respite stay at the facility. On or about August 4, 2019, the AV suffered 4 falls in one day, causing injury to his/her head and right knee. The facility failed to provide a safe environment to AV. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
