Findings: Alleged Victim (AV) was care planned for staff to ensure his/her oxygen was on and at the appropriate settings. AV experienced an episode where the oxygen was turned off, which caused AV's vitals signs to change causing AV unreasonable discomfort. The facility failed to follow AV's care plan which is a violation of resident rights, is neglect of care and constitutes abuse.
