Findings: Alleged Victim (AV) and Witness 2 (W2) had previous history of verbal and physical altercations. W2 was experiencing difficulty with AV’s decline and on or about August 26, 2019, W2 was attempting to assist AV with cares when they were involved in an altercation resulting in W2 hitting AV in the back of his/her head. The facility failed to implement appropriate interventions and provide a safe environment for AV, which are violations of resident rights, are considered neglect of care and constitutes abuse.
Sanction: ALFCP20-00274 $188.00 fine assessed
