Findings: The Alleged Perpetrator #2 (AP2) failed to perform adequate assessment and timely seek medical treatment when the Alleged Victim (AV) was experiencing a change of condition. Staff had urged AP2 to call emergency services earlier in the day. The AV fell while trying to stand after using the restroom, causing an abrasion above his/her eyebrow. The fall could have been prevented had emergency services been called. AP2' actions are a violation of residents rights, considered neglect of care and constitute abuse. The facility's failure to provide a safe environment is a violation of resident rights, is considered neglect of care and constitutes abuse.
