Findings: The facility failed to appropriately care plan and implement meaningful fall prevention for the Alleged Victim (AV) regarding his/her risk of falls. AV was found on the floor from a fall, was transported to the hospital for treatment and later that day, passed away. The facility failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
