Findings: The facility failed to provide appropriate care when the Alleged Victim (AV) experienced a fall and used his/her call light for assistance. According to documentation, it took approximately twenty-five minutes for staff to respond to AV's request for help after experiencing a fall. The failure placed AV at risk of harm, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
