Findings: Alleged Victim (AV) resides in a secured memory care unit for his/her safety and has a history of exit seeking behavior. On or about August 20, 2019, AV exited the facility and was unable to be located for approximately an hour. AV was found and returned to the facility by LEA, unharmed. The facility failed to provide a safe environment putting AV at risk for serious harm, which is a violation of resident rights, is neglect of care and constitutes abuse.
