Findings: Witness 1 (W1) and Alleged Victim (AV) have a known history of altercations. On or about July 4, 2019, AV and W1 got into an altercation resulting in AV experiencing unreasonable discomfort. The facility did not have enough staff to meet the scheduled and unscheduled needs of the residents and the facility failed to care plan appropriately according to W1's and AV's known history which is a violation of resident rights, is considered neglect of care and constitutes abuse.
