Findings: The facility failed to provide adequate supervision and implement appropriate interventions according to Witness 1’s (W1) known aggressive behaviors and history of altercations. On or about September 10, 2019, Alleged Victim (AV) and W1 were involved in an altercation, resulting in W1 grabbing AV’s wrist, causing AV to experience pain and fear. Days prior to this incident, W1 was involved in a similar altercation where W1 grabbed another resident’s wrist. W1’s care plan does not have sufficient instructions on how to monitor or prevent W1 from these behaviors. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
