Findings: The facility failed to implement interventions and provide adequate supervision according to Witness 3’s (W3) known aggressive behaviors. On or about September 11, 2019, Alleged Victim (AV) and W3 were involved in an altercation, resulting in W3 grabbing and twisting AV’s wrist. W3’s care plan does not have sufficient instructions on how to monitor or prevent W3 from these behaviors. The facility’s failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
