Findings: Witness 1 (W1) had a history of known aggressive behaviors and staff was to keep W1 in line of sight. On or about September 15, 2019, Alleged Victim (AV) and W1 were involved in an altercation in which W1 struck AV in the chest and head. The facility failed to ensure appropriate oversight and failed to provide a safe environment, placing AV at risk for harm which are violations of resident rights, are considered neglect of care and constitutes abuse.
Sanction: RCFCP20-00021 $375.00 fine assessed
