Findings: On or about September 4, 2019, Witness 1 (W1) hit Alleged Victim (AV) in the head when going into the dining room for breakfast. There was a concern that AV would have neck pain from having his/her head jolted. On or about September 9, 2019, W1's progress notes indicate W1 was being aggressive with staff, easily agitated and was being treated for a urinary tract infection. On or about September 10, 2019, W1 his AV hard on the back of the head again and there was concern that AV would have neck pain. The facility failed to care plan and implement appropriate interventions related to W1's known behavior, which is a violation of resident rights, is neglect of care and constitutes abuse.
