Findings: Alleged Victim (AV) requires staff assistance with transfers and uses a power wheelchair for ambulation. On or about September 19, 2019, Alleged Perpetrator 2 (AP2) was assisting AV with a transfer. AP2 moved AV's power chair forward fast, hitting a table. The commode did not fit in the area so AP2 moved the power chair backwards and hit AV, knocking him/her down causing pain and injury. AP2's actions are considered neglect and constitutes abuse. The facility failed to provide a safe environment which is a violation of Oregon Administrative Rules.
